Routinely GPs are caring for frail and ageing Australians with progressive chronic conditions, both malignant and non-malignant, that usually lead to an expected death. GPs are optimally positioned to provide the best possible care for this population of patients.
This workshop introduces a systematic approach for the implementation of quality end of life care for older Australians under the care of their GP in the community (home and aged care facilities). The approach is based upon a framework of care that utilizes patient choice, clinical knowledge and prognostic/needs assessment tools. It allows GPs to formulate proactive, patient-centred management plans that avoids decision making in emotionally charged situations. Using case based discussions the workshop focusses on common clinical challenges, difficult discussions, prescribing practices and potential medico legal dilemmas GPs need to navigate to achieve best practice patient outcomes.
This workshop introduces GPs to a wide variety of new resources available through Decision Assist including profession specific advance care planning and palliative care advice help phone lines, accredited palliative care education opportunities, a mobile phone app and monthly online palliative care case conferences.
